I grant permission to Upper Bathurst Animal Clinic to photograph my pet for the purpose of documenting the treatment and authorize Upper Bathurst Animal Clinic, its assigns & transferees to copyright, use and publish the same in print and/or electronically for clinic use. Confidentiality is assured.
